As we saw during the early days of Covid treatment, even state MD licensing dictates what a doctor can and cannot do.

My (now deceased) father-in-law used to go to people's houses in the 1950s and early 1960s to treat the patients who were unable to get to the hospital. He carried his black bag in the trunk of his car.

He retired when he was no longer allowed to treat patients as he saw fit.
Doctor: "I know this treatment will work, as I've done it several times with success on other patients. I'll write you a prescription for these 3 things."

Insurance Company: "Nahhh!"
